Squat Blooper - Definition of EPIC FAIL

So I was deloading with 185 and wanted to get some better form in. Straightened my back on the second rep but lost my footing. I instinctively lurched forward. The mat is pretty slippery since it is just a yoga mat I converted to a lifting platform.
Only person home is my Mom... Epic fail ensues. I was afraid to break the floor or something so I called her to help me out.
